Current students with merit scholarships requiring them to live on campus for two years can use this form to request a waiver from the second year of the requirement. Use this form if you have not yet signed a contract to live in the residence hall for the second year. If you have already signed a contract for the second year, use the Contract Release Request instead.

If your request relates to a physical, psychological, or medical condition, please contact Student Disability Services (SDS) to request an accommodation rather than submitting a waiver request. To preserve equity and confidentiality, requests of this nature must be approved by SDS.
