How to Get Started
- Review the FOM Manual: Familiarize yourself with the FOM system by reviewing the FOM Manuals.
- Create an account in FOM.
- Fill Out the FOM New Facility Request Form: Complete the form to request assistance for your facility's instrument management.
- Set Up Your Facility in FOM: Once approved, your facility will be integrated into
the FOM network, allowing you to start using the platform.
- Start adding resources to your facility in FOM.
- Collaborate with the IT team to ensure your facility’s needs are met and any technical requirements are addressed.
- If you wish to have your instrument access interlocked by FOM, please complete the Instrument Management Request form, once per instrument to be locked.

FOM Instrument Management Request
If your facility requires interlocks on equipment, please fill out the FOM Instrument Management Request Form. This request will be sent to MTU IT for processing. Once you have completed the form, the IT team will review your request and follow up with you as soon as possible.
