Registration forms help students gain necessary approval from their academic advisor or instructor, and register that approval with the Registrar's Office. Below are the procedures for each type of form, including a link to download a PDF of the form.
Procedures for Registration Forms

Grading Option Change Form
- Decide what grading option you would like:
- Audit
- Pass-Fail
- Download the Form
- Obtain Signatures:
- Academic Advisor
- Course Instructor
- Return the form to the Registrar's Office by the end of the add period.

Credit Overload Permission Form
- Decide how many credits you want to carry
- Download the Form
- Obtain Appropriate Signature:
- Academic Advior (Undergraduate or Graduate)
- Dean of Students (If you are on academic probation)
- Return the form to the Registrar's Office.

Approval/Waiver Form
- Find the restriction you would like to waive
- Download the Form
- Obtain Appropriate Signatures:
- Departmental Approval (From department offering course)
- Instructor
- Return the form to the Registrar's Office before registering for the class.
