The following is a list of resources and forms commonly used throughout the Laboratory Inspection program at Michigan Tech.

Pre-Inspection Templates

  • Hazard Risk Assessment: A hazard analysis or job safety analysis is required whether engaging in research or workplace activities involving hazardous materials or procedures.
  • Training Documentation / Log.  Training documentation is required for all laboratory processes and procedures, ensuring that any user understands the risks involved with the task, equipment, or process they are performing.  Training methods will vary.  
  • Standard Operation Procedures: Standard Operation Procedures are required, and describe each hazard and procedures to safely operate and mitigate them.             
  • Chemical Hygiene Plan: Laboratory use of hazardous chemicals means handling or use of such chemicals in which all of the following conditions are met:
      1. Chemical manipulations are carried out on a "laboratory scale;"
        1. "Laboratory scale" means work with substances in which the containers used for reactions, transfers, and other handling of substances are designed to be easily and safely manipulated by one person. "Laboratory scale" excludes those workplaces whose function is to produce commercial quantities of materials.
      2. Multiple chemical procedures or chemicals are used;
      3. The procedures involved are not part of a production process, nor in any way simulate a productions process; and
      4. "Protective laboratory practices and equipment" are available and in common use to minimize the potential for employee exposure to hazardous chemicals.
  • Additional hazard information may be required.  Additional EHS safety documents are available and may be required based on the hazard assessment. 
  • BioSafety Manual: A biosafety plan may be required if you work with biological materials, infectious agents / viruses, or toxic materials. Please review the link for more details. Note: BSL-2 laboratories require an IBC protocol.
