This page contains a listing of forms and deadlines for graduate students, faculty, and staff. Failure to meet submission deadlines could result in delayed completion of a student’s graduate degree. Current students should look to their personalized Degree Progress Checklist for a list of items they need to complete their degree.

For more information on the application process, please see our How to Apply page. Prospective students can track the status of their application on the Michigan Tech application portal. The online application is used by both domestic and international students to apply to any of our degree programs, certificate programs, or for non-degree seeking status.
The following are forms intended for International applicants that are in addition to the online application process.
Additional Forms for Admission

Submitting forms in a timely manner is essential for students to complete their degree requirements. Students can find a checklist of items needed at this time and a comprehensive list of items pending and complete for each degree on their Degree Progress Checklist. Forms for all degree seeking students (Masters/PhD)
- Patent, Research and Proprietary Rights Agreement— Submit on your Degree Progress Checklist
- Advisor and Committee Recommendation Form (not required for coursework students unless they want to change their advisor from the graduate program director)
- Degree Schedules and Transfer Credits— Submit one degree schedule per degree or certificate
- Transfer Credits Form—transfer credits from another university to Michigan Tech

Form for coursework students
Forms for students completing a dissertation, thesis, report
- Advisor Recommendation form—Submit to change or appoint an advisor. Coursework students only need to submit this form if they change advisors from their graduate program director. (new for fall 2024)
- Committee Recommendation form—Submit to appoint a committee. Note that your advisor is not listed on this form; they are automatically members of your committee. (new for fall 2024)
- Petition to Enter Candidacy (allows registration for research credits at the research mode rate)
- Notify the Graduate School of the date and time of your final oral examination ("Defense") on your Degree Progress Checklist. Please see our student and advisor tutorial for additional assistance.

- MIGS Transfer Form—Graduate students at Michigan Tech may take courses at another Michigan university with approval of both the home and host university

- Recording Graduate Exam Results - for programs to submit completion of qualifying or research proposal examinations. Replaces using SZAPROT in banner 8.

- Request for Graduate Tuition-Only Award/Fellowship
- Request to add a graduate student to a graduate program - a graduate program director may complete this form to admit a current certificate student into a master's program, or a current master's student into a PhD program

- Termination of Support Request - Request to stop support funding for student
- Update Graduate Staff - complete this form to update a graduate program director, assistant, or Graduate Faculty Council representative
All Graduate School forms work best in the software that created them. PDF forms work best in Adobe Acrobat or Adobe Reader; DOCX files work best in Microsoft Word. If your browser doesn't display a form when you click on the link, your browser is using an incompatible plug-in to open the file. IT has created a service desk article illustrating several ways to open PDF forms. If you are experiencing problems try:

August 21, 2024 - Wednesday before classes start
Grace period deadline
Last day to submit committee approved post-defense dissertation, thesis, or report to the Graduate School to the Graduate School to qualify for enrollment in UN5951 and degree completion this semester.
- Students who meet this deadline will be eligible to graduate this semester. The Graduate School will enroll students in UN5951. Students will pay the resubmission fee when their document is accepted.
- Students who do not meet this deadline will need to maintain continuous enrollment until they complete their degree.
September 27, 2024 - Friday, Week 5
Submit your Graduation Application if you are completing a master's or PhD this semester.
December 2, 2024 - Monday, Week 14
Last day to submit committee approved post-defense dissertation, thesis, or report to the Graduate School for fall completion of degree.
- Students who meet this deadline will be eligible to graduate this semester, pending Graduate School review. Please allow six business days for review of documents. Students will have until the Friday of Finals Week to complete any formatting changes required by the Graduate School.
- Students who do not meet this deadline will not be eligible to graduate this semester and will need to maintain continuous enrollment until they complete their degree.
December 2, 2024 - Monday, Week 14
- Submit all other degree completion paperwork (including applicablecertificate degree schedules) to Graduate School—check your Degree Progress Checklist for items needed.
- Submit petition to enter candidacy to be eligible for candidacy for next semester.
January 2, 2025 - Thursday before classes start
Grace period deadline
Last day to submit committee approved post-defense dissertation, thesis, or report to the Graduate School to qualify for enrollment in UN5951 and degree completion this semester.
- Students who meet this deadline will be eligible to graduate this semester. The Graduate School will enroll students in UN5951. Students will pay the resubmission fee when their document is accepted.

February 7, 2025 - Friday, Week 5
Submit your Graduation Application if you are completing a master's or PhD this semester.
April 14, 2025 - Monday, Week 14
Last day to submit committee approved post-defense dissertation, thesis, or report to the Graduate School for spring completion of degree.
- Students who meet this deadline will be eligible to graduate this semester, pending Graduate School review. Please allow six business days for review of documents. Students will have until the Friday of Week 15 to complete any formatting changes required by the Graduate School.
- Students who do not meet this deadline will not be eligible to graduate this semester and will need to maintain continuous enrollment until they complete their degree.
April 14, 2025 - Monday, Week 14
- Submit all other degree completion paperwork (including applicablecertificate degree schedules) to Graduate School—check your Degree Progress Checklist for items needed.
- Submit petition to enter candidacy to be eligible for candidacy for next semester.
April 30, 2025 - Wednesday Before Classes Start
Grace period deadline
Last day to submit committee approved post-defense dissertation, thesis, or report to the Graduate School to the Graduate School to qualify for grace period submission for degree completion this semester.
- Students who meet this deadline will be eligible to graduate this semester. The Graduate School will enroll students in UN5951. Students will pay the resubmission fee when their document is accepted.
- Students who do not meet this deadline will need to maintain continuous enrollment until they complete their degree.
June 6, 2025 - Friday, Week 5
Submit your Graduation Application if you are completing a master's or PhD this semester.
August 4, 2025 - Monday, Week 14
